Stimulation of fracture Healing by Low-Intensity Pulsing Ultrasound

We performed a preliminary study to evaluate the effect of ultrasound in canine osteotomy model. Both ulna shafts were osteotomized and one side was irradiated with low-energy pulsing ultrasound, 50mW/cm2, 1 MHz, 200 microsecond bursting sine wave for 15 minutes, 6 days a week. Effect on the fracture healing was evaluated by radiologic scoring system, amount of uptake of radioactive isotope, and time to union. Results were as follows : At postoperative 2 months, means of the radiologic healing score were 4.5 in the irradiated side and 2.5 on the control side (p=0.019). Mean isotope uptake in the irradiated side was 44.4 count and 33.8 count in the control side (p=0.028). Two nonunion developed on the control side. Mean time to union of eight dogs was 2.6 months in the irradiated side and 3.1 months on the control side. Based on the cumulative union rate curve, fracture healing was stimulated and union was obtained one month faster in the irradiated side than the control side.

Treatment of Nonunion Using Electrical Stimulation Only

Negative electric charge is known to induce osteoblastic activity in bone. Therefore, electrical stimulation can be used clinically to promote more active new bone formation in the fracture site. Among many methods in the treatment of fracture nonunion, electrical stimulation can be emplyed as one of the useful methods, which is less of noninvasive and rather simple. We experienced 10 cases of fracture nonunion managed with EST without surgery in the Department of Orthtopedic Surgery of Seoul National University Hospital from September 1980 to March 1987. Of these, 6 cases were managed with semiinvasive direct current method and resulted in union of all cases. 4 cases were managed with nonivasive capacitively coupled electric field method and resulted in union iin 3 cases but in1 case, nonunion persisted till 22 months follow-up. In the 9 cases of satisfactory results, union time varied from 2 months to 9 months with mean of 4 months. No remarkable complication was found in these cases. In conclusion, electrial sitmulation therapy is thought to be one of the useful and hopeful methods in the treatment of fracture nonunion cases in which surgery is not absolutely indicated.

Treatment of Intractable Nonunion of the Long Bone with the Invasive Electrical Stimulation

Nonunion of the long bone is one of the difficult porbloems in orthopaedic surgery. We studied the effect of the electrical stimulation. From July, 1980 to August, 1988, 30 nonunions of the long bones were treated with the invasive type electrical stimulator. The range of follow-up period was from 3 months to 7 years(average, 25 months). The results were as foloowings: 1. The good bony union has occurred in 26 cases(86.7%). 2. The previous infection was in 11 cases and its union rate was 100% 3. The average duration of bone union in over-all cases was 5. months and in previous infeted cases, was 5.5 months. 4. The average duration of bone union in open fracture was 5.5 months and in closed fracture, was 5 months. 5. The invasive electrical stimulation was a good technique to treat the intractable nonunion as the result of long bone fracture.
