PURPOSE To analyze the factors predisposing to complications after internal fixation of femoral neck fracture. MATERIALS AND METHODS We reviewed retrospectively the results of percutaneous internal fixation of femoral neck fracture using multiple pinning, in 52 cases who were treated from Jan. 1996 to Dec. 2001. Relationship between the complications and several factors such as the age, sex, time interval from injury to operation, Garden stage, Singh index, internal fixation device and state of redction were analyzed. RESULTS The functional results by Lunceford criteria were excellent in 23 cases (44%), good in 15 cases (29%), fair in 2 cases (3.8%) and poor in 12 cases (23.1%). The avascular necrosis of the femoral head were occured in 14 cases (26.9%). Among these, 1 case of non-union, 2 cases of mal-union were accompanied. No stastically significant relationship between the age, sex, time interval from injury to operation, Garden stage, Singh index, internal fixation device, state of redction and complication. However, there was 4 times higher complication rate in Garden stage 3 or 4 group than its rate in Garden stage 1 (odds ratio 3.889), and 3 times higher complication rate in non-anatomical reduction group (odds ratio 3.22). CONCLUSION Factors predisposing to complications after internal fixation of femoral neck fracture seemed to closely relate with Garden stage and state of reduction.

PURPOSE The purpose of this study is to analyze the clinical and radiographic prognostic factors which may affect the postoperative clinical results of the unstable ankle fractures. MATERIALS AND METHODS This study is based on 75 unstable ankle fractures treated by open reduction and internal fixation from May 1994 to August 2000, with a minimum follow-up period of 12 months(range : 13 months-7 years 3 months). The 75 patients were average 40.5 years old with male: female ratio of 52:23. Based on Lauge-Hansen classification, the supination-external rotation type was the most common with 42 (56.0%) cases. The clinical results was assessed by American Orthopaedic Foot and Ankle Society(AOFAS) functional scale. The sex, age, side of injury, body weight, trauma-operation interval, operation time, cause of injury as the possible postoperative clinical prognostic factors and fracture type, anatomical reduction of fracture, preoperative medial clear space, postoperative medial clear space, talo-crural angle, talar tilt, tibio-fibular clear space, tibio-fibular overlap space as the possible radiographic prognostic factor were statistically analyzed RESULT: Postoperative AOFAS functional scale was average 81.0 points with 23(30.7%) cases excellent, 17(22.7%) good, 18(24.0%) fair and 17(22.7%) cases poor results. The age, the operation time(p<0.001) and the anatomical reduction of fracture(p<0.005) were found to be statistically significant factors affecting the prognosis. The other clinical and radiographic factors did not significantly affect the clinical results. CONCLUSION The surgically treated unstable ankle fractures in patients whose age was above 41 years old or operation time exceeding 90 minutes or unsatisfied anatomical reduction of fractures showed significantly poor clinical results.

PURPOSE Monteggia fracture is a combination of ulnar fracture and radial head dislocation. Despite of low incidence, Monteggia fracture has a problem in the treatment. So satisfactory results could not be achieved easily. The purpose of this study was to analyze factors affecting functional results in Monteggia fracture. MATERIALS AND METHODS From January 1990 to June 1998, we retrospectively reviewed 24 patients out of 40 patients who had been treated for Monteggia fracture. The functional assessment was performed with Broberg and Morrey system10). RESULTS Excellent and good results were achieved in 16 cases(66.6%), whereas fair and poor results in 8 cases(33.4%). Satisfactory results were obtained in 66.6% overall.
According to the Bado classification, functional results were not different significantly between types. According to the mechanisms of injury, unsatisfactory results were obtained in 8 of 17 patients with high energy injury. Monteggia fractures with ulnar fracture at the diaphyseal level showed satisfactory results significantly compared to others. In terms of the methods of internal fixation, the plate and screw fixation showed satisfactory results in 14 of 16 patients whereas tension band wiring or intramedullary nailing showed unsatisfactory results in 6 of 8 patients. CONCLUSION In Monteggia fractures, factors affecting functional result may be the mechanism of injury, the level of ulnar fracture and the method of internal fixation.

The calcaneus is the most commonly fractured tarsal bone, but the appropriate care of calcaneal fracture continue to be an unsolved dilemma. As technology in imaging has improved, operative treatment is more suggested. The purpose of this study is to evaluate the results of operative treatment in intra-articular calcaneal frcture and to analyse the results in accordance with various prognostic factors. We analysed retrospectively 13 patients, 17 intra-articular calcaneal fractures undergone operative treatment. Mean follow-up period was 27 months (range:13 ~ 44 months). There were 11 males and 2 females with 41 year old mean age (range:18 ~ 63 years old). Clinical assessment used the modified Creighton-Nebraska health foundation assessment sheet for fracture of the calcaneus. We obtained excellent result in 7 cases (41.2%), good in 2 cases (11.8%), fair in 7 cases (41.2%) and poor in 1 case (5.8%). Clinically age and body weight, radiologically Bohler angle, fibulo-calcaneal distance and subtalar joint discrepancy are related to the prognosis of intra-articular calcaneal fracture following operative treatment. Postoperative complications are limping (2 case), heel pain (3 cases), hump bump of calcaneus (1 case) and subtalar arthritis (1case). In conclusion, on the basis of our results, there is a relationship between anatomical abnormalities of the heel and a poor clinical outcome. Therefore, in operative treatment of intra-articular fracture of calcaneus, we recommend anatomical reduction, if possible, not only of the subtalar joint but also of the Bohler angle and fibulo-calcaneal distance.
Forty-six patients who had had forty-nine calcaneal fractures were managed with closed reduction and a cast, closed reduction and axial pin fixation, and open reduction and internal fixation. The results were reviewed retrospectively, between twelve months and twenty-six months after the treatment, with criteria usued in assesssment of result by Salama30) and with plain radiographs. The purpose of this study were to analyze the relationship between the prognostic factors and post-treatment results. The results of treatment and prognostic factors obtained from this study were as follows. 1. The negative prognostic factors that were associated with an unsatisfactory out come were an age of more than fifty years, a greater body weight, work involving strenuous labor, subtalar incongruity, a decreased fibulocalcaneal space, and a decreased B hler-angle ratio of the fractured to the normal side. 2. Patients who had had a tongue-type fracture had a better result than those who had had a central depression fracture, while those who had had a central depression fracture had a better outcome than those who had had a comminuted fracture. 3. The length of the Achilles-tendon fulcrum, which is directly related to the calcaneal length, was not related to the outcome.

Most authors report 70% to 80% good to excellent results following open reduction and internal fixation of fractures of the patella. As with most intraarticular fractures, the prognosis for healing and restoration of function in fractures of the patella is dependent on the amount of articular cartilage damage and exact reduction of fracture.
In order to document clinical results and describe any prognostic factors, we evaluated the results of 69 patellar fractures treated in 66 patients who had been treated with open reduction and internal fixation at the Department of Orthopaedic Surgery, Nationanl Medical Center, from January 1990 to December 1994.
The average age at the time of surgery was 41.2 years(16 years-75 years) and the average follow-up time was 38.7 months(14 months-74 months).
The results were as fellows: 1. The mechanism of injury(traffic accident), concomittant inury of ipsilateral lower extremity, fracture morphology, immgbilization period less than two weeks and accuracy of reduction were significant prognostic factors.
2. Age and method of fixation were not significant prognostic factors.
3. The brisement of knee under general anesthesia was helpful, that had been performed following bony union.
